Non NHS Examinations and Reports
Some services provided are not covered under our contract with the NHS and therefore attract charges.
Examples include the following:
- Medicals for pre-employment, sports and driving requirements (HGV, PSV etc.)
- Insurance claim forms
- Prescriptions for taking medication abroad
- Private sick notes
- Vaccination certificates
These services are only available outside of normal appointment times.
There is a charge for all non-NHS services. Please be prepared to pay at the time you receive the service. Please contact the surgery to enquire whether you need an appointment and how payment is made.
Some things you should note:
- Our NHS services always take priority and non-NHS services are provided subject to GP availability. At peak times, we reserve the right to refuse to undertake non-NHS services. Please allow at least 4 weeks from the submission of any request.
- Our GP’s will not sign passports or any other documents which require them to provide personal details such as home telephone numbers, addresses etc;
- Forms and reports are done in order of date receipt and not in order of magnitude.
- We do not undertake DNA testing.
- We do not undertake HIV testing.

- Foster Medical: £130
- Foster Assessment Form: £50
- Holiday Cancellation Form?; £50
- Private Letter: £50
- Accident/Sickness Insurance Form: £50
- HEP B - Full Course: £150
- Medical Records Extracts including Data Protection requests: First copy of records is free of charge, any additional repeat copies requested will attract a fee, chargeable per page.
