New Patients

To register with our practice:

• Please visit our reception desk and inform the receptionist that you wish to register with the practice.  The receptionist will ask what your current address is, this is to ensure that your reside within the practice boundary.  The practice boundary is from Stockhams Corner roundabout back to Sunnycroft Roundabout (by the Bagle Brook Beefeater Restaurant).
• You will be given a registration form (this allows us to request your medical records from your current practice) along with our practice leaflet.  Please click on the link below to download a copy of the registration form.
• You need to complete the registration form and return it to reception along with a form of photographic ID such as a driving licence, bus pass, passport etc and a form of proof of address which we require to be a recent utility bill (dated within the last couple of months).  If you are currently on any repeat medications, we would also need a list of these (preferable a copy of your repeat prescriptions form).  We can then ensure they are on the system ready for you to order when needed.
• A new patient review will also be booked for you with our health care support worker.  This is just for us to know if you have any health conditions we need to be aware of prior to us receiving your medical records and for us to record your current height, weight, blood pressure etc.
